Our practice contracts with many insurance companies. Please contact our office to verify acceptance of your insurance plan.
Many insurances require a medical provider's prescription or a referral which must be sent ahead of your appointment time or be brought in with you.
Insurance coverage and the amount owed by the patient may differ based on your particular plan. We are "In Network" with most plans but "Out of Network" with a few which may mean the patient pays more in the way of deductible, copay or coinsurance.
We ask for your insurance information ahead of time to give us the chance to verify your benefits and inform you of such things as the number of visits allowed, your remaining deductible if any, and your copay (a set dollar amount) or coinsurance (a percentage of what insurance allows) which is due each visit.
Patients must pay each visit for copays, and deductibles until met. We will give you an estimated amount for your coinsurance each visit which will be due at the time of the visit. We will ask for additional amount or reimburse you if the estimate is more or less than your insurance pays.
We recommend that the patient also checks their insurance plan benefits in case we are given incorrect information through an insurance company website or employee.
We are happy to file insurance claims for your physical therapy.
